I don't participate directly in insurance panels, but I'm happy to provide a "superbill" upon request, a receipt containing everything your insurance company needs to consider out-of-network reimbursement. I'd recommend calling your insurance company to ask whether you qualify for partial out-of-network reimbursement, and to confirm they'll accept telehealth sessions, since that's how all of my sessions are currently conducted (this is noted on the superbill). If you do have out-of-network benefits, many clients find the app Reimbursify helpful for submitting superbills and tracking reimbursement.
My session fee is $300 for a 45 to 50 minute session, due at the time of your appointment.

If you're submitting a superbill for out-of-network reimbursement, your insurance company may ask for the following details:
Provider: Katerina S. Levy, Psy.D.
License: NY State Licensed Clinical Psychologist, License #028397
Common CPT Codes: 90791 (initial diagnostic evaluation), 90834 (45-minute individual psychotherapy), 90837 (60-minute individual psychotherapy)Your superbill will include the specific code used for each session, along with the diagnosis code, date of service, and fee, so you generally won't need to track this down yourself. It can still be helpful to have on hand when you first call your insurance company to ask about your out-of-network benefits.

As a New York State licensed psychologist, I can provide telehealth sessions to anyone physically located within New York State at the time of our appointment, regardless of where they otherwise live.

My practice spans the lifespan. I work with children, adolescents, and adults, including individuals and families managing chronic illness, anxiety, depression, and major life transitions.
-
I draw primarily on evidence-based approaches, including Cognitive Behavioral Therapy (CBT), Acceptance and Commitment Therapy (ACT), and Trauma-Focused CBT (TF-CBT). Treatment is always tailored collaboratively to each person's goals and needs rather than following a one-size-fits-all model.

Sessions typically run 45 to 50 minutes. Most clients start with weekly sessions to build momentum and a strong working relationship, and we adjust frequency together as treatment progresses.

I'd love to hear from you. Reach out through the contact form or by phone at (917) 765-6922, and we'll set up a brief consultation call. This gives us a chance to talk through what's bringing you to therapy and answer any questions you have, before scheduling your first session together.
